Comments for 10/16/14 submittal:

1.) Identify all haul routes, demolition disposal and material sources, including frequency and types of proposed maintenance of haul routes and emergency telephone numbers and contact persons.  List the days and hours of haul route use and submit traffic control plan, if required.  The shortest, most direct route, which minimizes travel on local and collector roadways shall be used. 

The selected general contractor shall provide haul route information prior to issuance of permit. 

2.) This review shall be considered incomplete pending receipt and approval of the requested revisions or additional comments.  10/16/14 ~DF~
