1. Please provide statement of special inspection per AMC 23.15.1704.2.3 and 1704.3.1.
Please include special inspection for installation of post installed column bases.
2. Please provide letter of special inspection from agency hired to perform special inspections.
3. Please show on plans approximate amounts of settlement and how many inches foundation will be raised to reach level.
4. Provide structural calculations for new GLB.
5. CB66 bases are designed to handle uplift forces of nearly 4000 lbs per Simpson allowable uplift load in cracked concrete for seismic design
category D. How will the tie between old concrete and new grout handle these loads? Please provide a detail showing exactly how CB66 bases will beinstalled. Show extent of drilling and grouting, and include adhesive and/or adhesive anchors/epoxy rebar dowels in existing concrete to prevent uplift of
newly grouted CB66.
6. Please provide structural calculations for loads on helical pile foundation. Soils letter states helical piles are designed with a factor safety of two.
Please provide backup including size of loads on piles and demonstrate adequacy of piles to resist load. Provide calculated pile loads atall locations. If using approved MOA policy for helical piles, S.08 or S.09, please provide note to clarify.
7. If only a portion of the foundation will be supported by helical piles, has future differential settlement been considered?
Please address all comments and provide submittals as necessaryto the permit manager. Further review may be required.
