1. Drainage Report:  If applicable complete the Drainage
Certification found in the Drainage Design Guidelines Appendix C-3

) , along with backup calculations (areas and
weighted coefficients) stamped by a civil Engineer (
http://www.muni.org/Departments/works/project_management/Pages/Publications.aspx).
Otherwise, Provide a drainage report prepared, signed and stamped by a civil
engineer licensed in the State of Alaska per the 2007 DCM Ch:2.  Reference
the 2007 Drainage Design Guidelines, Appendix A for preparation of a Drainage
Report (follow the outline beginning on page A-3 and include relevant forms) to
be submitted with the drainage and grading plan.  Please provide drainage
calculation inputs and results from modeling on a CD upon request (EPA SWMM
digital format or equivalent as applicable). Resolved with form C-3, MG 10-4-18.



2. Ref. C3.0, Proposed connection to 60” pipe; A manhole shall be installed at major junctions places where there are
changes in vertical or horizontal alignment, and at locations where there are
changes in pipe size or shape.  2007 DCM 2.7.D.1. Resolved per email from
Paul VanLandingham dated 9-25-18, MG 10-4-18.



3. Ref. C3.0, Proposed connection to 60” pipe; Two 12” dia. pipe inverts are shown
and only one 60” pipe invert. This appears to be an error.



4. Indicate the installation of storm drain pipe locator tape three feet below
finish grade or two feet deep in the street structural section. MASS Section
20.13 Article 13.3.E, page 39 R2. Resolved, MG 10-4-18.
