3.Submit an Operation and Maintenance Manual for the permanent storm water treatment controls. See section 8, STORM WATER TREATMENT GUIDANCE MANUAL 3RD EDITION: for this requirement the Plan shall provide more specific guidance for maintenance, operations and inspection of the permanent BMPs, Provide periodic inspection of the parking lot for spills and debris. The parking lot is required to be swept twice per year, By June 15th and Oct 15th. Permanent BMPs are required to be cleaned on the same schedule. 5.Submit a copy of the approved filed APDES Notice of Intent (NOI)

For construction project disturbing one or more acres or less than one acre but part of a larger common plan of development, the MOA is required under the MS4 permit to verify whether the construction site operator(s) have obtained necessary coverage under the APDES Construction General Permit before issuance of a storm water review approval.
