Comments for 7/9/14 submittal:

1.) Identify demolition disposal and materials source haul routes, including frequency and types of proposed maintenance of haul routes and emergency telephone numbers and emergency contact persons.  List the days and hours of haul route use and submit traffic control plan, if required.  The shortest, most direct route, which minimizes travel on local and collector roadways shall be used. 

2.) This review shall be considered incomplete pending receipt and approval of the requested revisions or additional comments.  7/16/14 ~DF~
