Comments for 11/21/13 submittal:

1.) Identify all haul routes for demolition, material sources and disposal sites including frequency and types of proposed maintenance of haul routes and emergency telephone contact persons.  List the days and hours of haul route use and submit temporary Traffic Control Plan (TCP) for sidewalk work on 4th Avenue and the hydrant, water service and sewer installation operations along "G" Street and the unnamed alley.  The shortest, most direct route, which minimizes travel on local and collector roadways shall be used. 

2.) Provide letter on non-objection or statement of concurrence from the Anchorage Community Development Authority (ACDA) regarding the on street metered parking spaces, which will be temporarily removed due to the replacement of a segment of the public sidewalk along 4th Avenue. 

3.) Provide sign schedule and installation detail for the existing signs or appurtenances to be removed and replaced for construction of the proposed heated aggregate sidewalk on 4th Avenue. 

4.) This review shall be considered incomplete pending receipt and approval of the requested revisions or additional comments.  11/27/13 ~DF~
