Advisory Information:

1. Provide NOI and SWPPP.  SWPPP PROVIDED.  RESOLVED SME 5-23-12.

2. Complete an AG 21 checklist 2 and 3.  Provide all required documents for a project of this size. RESOLVED SME 5-23-12.

3.  Provide an operations and maintenance manual for this site. RESOLVED SME 5-23-12.

4.  Provide a storm drain as-built/inventory certified by a PE prior to receiving a final CO.

7. Provide an operations and maintenance manual for this site.  The O&M manual provided to Private Development does not meet the storm water requirements.  The O&M manual needs to address parking lot sweeping 2 times per year (NLT June 15th and Oct 15).  It also needs to provide for periodic cleaning of spills and debris on the parking lot. Address periodic maintenance of Pond 1.  How is it to be maintained and on what frequency.  How will the owner determine if the pond is still infiltrating at the required minimum rate?  The O&M manual should be in a letter format from the Engineer to the owner or from the owner to the MOA. 

8.  The field version of the SWPPP needs to contain a copy of the 2011 CGP.  A summary of the CGP as contained in Appendix Q, does not meet the requlatory requirements.  See the CGP, Part 5.3.5, Site Map(s).  The SWPPP needs to contain a map or maps meeting the requirements of this part.  I believe you can still keep these maps as part of the plan set as long as the SWPPP refers to them and they meet the requirements.

9.  How will the limits of disturbance be marked especially in the areas of the Glenn Highway Screening landscaping?

11. Based on the information in the geotechnical report do you anticipate excavation dewatering? If it is anticipated please complete a dewatering request form.

12.  This permit requires monthly inspections.  Total NPDES inspection fees are $1600 per year.  An additional $1000.00 may be required.
