Comment: * ATTENTION: The following discharges to the Municipal Separate Storm Sewer Systems (MS4) (stormwater conveyance such as but not limited to, storm drains, pipes, ditches, etc.) is prohibited unless otherwise authorized in writing by the ADEC. * Sewage; *Discharges of wash water resulting from the hosing or cleaning of gas stations, auto repair garages, or other types of automotive services facilities; *Discharges resulting from the cleaning, repair, or maintenance of any type of equipment, machinery, or facility, including motor vehicles, cement-related equipment, and port-a-potty servicing, etc.; *Discharges of wash water from mobile operations, such as mobile automobile or truck washing, steam cleaning, power washing, and carpet cleaning, etc.; *Discharges of wash water from the cleaning or hosing of impervious surfaces in municipal, industrial, commercial, and residential areas – including parking lots, streets, sidewalks, driveways, patios, plazas, work yards and outdoor eating or drinking areas, etc. – where no detergents are used and no spills or leaks or toxic or hazardous materials have occurred (unless all spilled material has been removed); *Discharges of runoff from material storage areas containing chemicals, fuels, grease, oil or other hazardous material; *Discharges of pool or fountain water containing chlorine, biocides, or other chemicals; discharges of pool or fountain filter backwash water; *Discharges of sediment, pet waste, vegetation clippings, or other landscape or construction-related wastes; and *Discharges of food-related wastes (grease, fish processing, and restaurant kitchen mat and trash bin wash water, etc.). The grading and drainage work appears complete and OK for CO. MG 9-30-20. Re: Buildings A & B; OK for CCO* until 7-15-20, All landscaping (established vegetation) & swales to be completed and inspected prior to 7-15-20. Request a Private Development inspection via computer & phone (to arrange time and date of inspection call 343-8115) prior to 7-15-20. *Abraham Gallo indicated that Chugach Electric tore up the landscaping, during the frozen 12-11-19 inspection. Re: Building A only: OK for Building A CCO, MG 7-5-19. The following must be resolved prior to issuance of a CCO beyond 7-15-20 (building A & B – lot 39): Resolved, MG 9-30-20. 1. The recordation of the O&M Agreement (includes O&M Plan). Resolved, MG 7-5-19. 2. Submittal of the Civil Work Asbuilt. Resolved, MG 7-5-19. 3. SDMH#1 – This manhole (stormceptor) was not found during the inspection. Inspection incomplete. Resolved, MG 7-5-19. 4. SDMH#2 – The rim elevation is high and does not receive drainage as designed. This item is partially resolved per the letter from the engineer dated 7-3-19 and is expected to be completely resolved upon final inspection. OK for CCO of building A only, MG 7-5-19. To be reinspected once landscaping is complete, MG 12-11-19. Resolved, MG 9-30-20. The following must be resolved prior to issuance of a CO (buildings A & B) or CCO (building B – lot 39): 5. Complete landscaping of swale (east of building A) with lead to CB#4. To be re-inspected once landscaping is complete, MG 12-11-19. Resolved, MG 9-30-20. 6. Complete grading and landscaping of swale (west of building B) with lead to SDMH#2. To be reinspected once landscaping is complete, MG 12-11-19. Resolved, MG 9-30-20. 7. Complete landscaping. To be re-inspected once landscaping is complete, MG 12-11-19. Resolved, MG 9-30-20. 8. Replace damaged field inlet casting on SDMH#2 and CB#4. Resolved, MG 7-5-19. 9. Complete grouting of CB#3 grade rings & oversize SD pipe opening, MG 7-3-19. Resolved, MG 7-5-19. 10. Inspection incomplete as of 7-1-19. To be re-inspected once landscaping is complete, MG 12-11-19.
